How do I make an appointment?
Simply call the office and someone will be glad to assist you. Office staff will be able to guide you through the steps and make an appointment in as timely a manner as possible. Please have your medical insurance card available. We will verify your insurance before you come in and let you know if a referral is required before you can see a specialist.
What do I need to bring to my appointment?
Please bring your medical insurance card, a photo ID, any recent imaging studies you’ve had done for the problem Dr. Nolan will be treating (i.e. X-rays, MRI, CT scans), including the reports. If you don’t have any, that’s ok. We can x-ray you here in our office.
Do you have Spanish speaking translators available?
Yes. Both offices have staff that can assist Spanish speaking patients. All other foreign patients will need to bring a translator with them.
Does Dr. Nolan take Worker’s Compensation injuries?
Yes. Just make sure you have completed the following steps:
- Reporting the injury as soon as possible to your employer.
- They will contact their insurance carrier and issue you a claim number and name of an adjuster.
- Please give us this information along with date of injury and contact information for your adjuster.
Does your office complete medical disability forms?
Yes. Our office staff does complete short term, long term and FMLA disability forms. We ask that you allow several days for completion. You may contact our office for further information.
What happens when I arrive for my appointment?
On arrival, you will be asked to sign in and present your insurance card and photo ID. Copies of these will be made for your chart. For your convenience, these forms are available online. If you would like to fill out these forms before you come in for your appointment, please click here.
How do I refill a prescription?
If Dr. Nolan has prescribed medication for you and you require a refill, simply contact your pharmacy. They will contact our office for the doctor’s approval. Please allow 24-48 hours for your refill if possible. Refills are not approved over the weekend.
